THE FOLLOWING DOCUMENTS ARE REQURED PRIOR TO FILING ANY BANKRUPTCY:
1. GET COPY OF A FREE CREDIT REPORT & FAX OR EMAIL TO ME. You may obtain a copy of your credit report at annualcreditreport.com;
2. Past three years tax returns (Federal and State) with W-2s. NOTE: If you cannot locate these returns, call the IRS at 1-800-829-3676 or 1-800-829-1040 and ask for a "transcript" to be faxed or mailed to you;
3. Provide copies of at least two months’pay stubs (complete set);
4. Copy of the last three months of Bank Statements. If you are self-employed, provide six months’ bank statements of both business and personal accounts.) Note: if you have multiple accounts, I need to see copies of each accounts
Note: I will need a statement from the day your case is filed. I will check with you on the day your case is filed to find out the checking/savings balance. Please get me a copy of this statement within 3 days. The Chapter Trustee will continue your case if this statement is not included.
5. Copy of County tax appraisal (note if you live in Richland/Lexington County, I will try to do this for you);
6. Copy of any 401(k) loan documents;
7. Copy of any family court settlement agreement from the past five years;
8. ***Credit Counseling Certificate. My experience has been that Hummingbird Credit Counseling Agency has been the easiest to use. You may contact them at http://www.hummingbird.org/, or by calling 1-800-645-4959. By law, credit counseling agencies must provide this service for free to those who cannot afford it. Ask them if you qualify for a free certificate. Finally, give them my fax #(803) 407 3345 to fax the certificate once you have completed the course.
